Incident Assignments and Performance Ratings of Michael Karl McNeil

Three written performance ratings of Michael Karl McNeil survive from incident assignments between 2000 and 2007. Each is summarized here as it reads, including the one that is mixed. Raters are identified by initials, rank and type of agency.

7 June 2007: Incident Commander Type 4

Form: Incident Personnel Performance Rating, ICS-225. Rated by G.R., a federal fire Incident Commander Type 3. The form’s scale runs from 0, deficient, to 3, superior, which it defines as “Employee consistently exceeds the performance requirements.”

He was marked 3 on both hot line and mop-up for ability to obtain performance, attitude, decisions under stress, initiative, consideration for personnel welfare, obtaining necessary equipment and supplies, physical ability for the job, and safety. Knowledge of the job was marked 2, satisfactory. The remarks block is blank.

December 2003: Dozer Boss and Line Scout

Form: Individual Performance Rating, ICS-226, signed 5 December 2003. Rated by P.S., an Incident Commander from a state forestry unit, after a four-day assignment of complexity II.

Scouting line for a future burning operation was rated Exceeds Successful. Constructing line with assigned dozers, and directing equipment into the appropriate location, were each rated Fully Successful. The rater’s remarks, exactly as written:

“Mike was willing to take on the assignment of locating 2 CDF dozers, Redirecting them from their previous assignment. Scouting out and flagging a location for the new dozer line that better supported the firing operation. Mike then led the dozers through the placement of this line. The line placement was well thoughtout and completed quickly.”

May 2000: Initial Attack Incident Commander

Form: Fire Assignment Evaluation, FC-121, dated 3 July 2000. It was completed by P.S., a state Fire Captain who was his immediate supervisor on the assignment and is a different person from the 2003 rater. He had been dispatched as a single resource for two days in May 2000.

Performance in the role was marked Average and attitude Outstanding. The box “employee needs more experience” was ticked. Problems encountered: None. It is the earliest of the three ratings on file, and it is reported in full.
